Reconstructive and Regenerative Therapy of Atrophic Jaws with New Implant Techniques: Preclinical and Clinical Studies

چکیده
Nowadays, dental implants represent a reliable and successful procedure for the prosthetic restoration of partially and totally edentulous patients, with high survival and success rates in the short, medium, and long term. However, a sufficient amount of bone—in terms of height and width of the residual bone crest—is needed to place dental implants in the proper position and inclination, to allow the placement of a biologically, functionally, and aesthetically integrated prosthetic restoration. Therefore, in the last years, several bone regeneration techniques have been developed, to allow proper placement of dental implants and the successful prosthetic restorations of patients with different types of bone defects. Among these techniques, there are onlay/inlay block regeneration, guided bone regeneration (GBR) with membranes, split crest techniques, maxillary sinus augmentation, and many others. The objective of the bone regeneration is to promote the formation of new bone, in order to reconstruct an atrophic alveolar ridge before or in conjunction with implant placement, through the use of different biomaterials (autografts, allografts, xenografts, and synthetic materials) alone or in conjunctionwith biostimulants (growth factors or stem cells). At themoment, in the international scientific community, there is great interest in the surgical techniques used for bone regeneration, from the most conventional (x example, bone regenerationwith autografts or allografts blocks, or GBRwith membranes) to the most modern and revolutionary (such as regeneration with custom-made synthetic scaffolds obtained with the modern digital technologies). Moreover, bone regeneration is related to technological development and the discovery of new materials. For example, the biological regeneration with platelet concentrates is today another important interdisciplinary field of research, in which engineering principles and basic sciences are used to develop biological substitutes that can repair and regenerate the function of bone tissue damaged by trauma, degenerative diseases. Today, some progress has been made in specific surgical applications, such as GBR andmaxillary sinus augmentation, that represent safe and predictable treatment procedures; the real “last challenge” of biomaterials research in dentistry appears to be the vertical bone regeneration through onlay blocks of different materials, supported by a valid blood perfusion that can guarantee that “biological push” that eases the regeneration process within the entire block. In the present special issue you will find a collection of articles dealing with different strategies for the bone regeneration in dentistry and maxillofacial surgery.

AN OVERVIEW OF STEM CELL AND REGENERATIVE MEDICINE RESEARCHES IN IRAN
Background: In many years, finding modern and appropriate treatments for diseases, especially chronic diseases (such as chronic metabolic diseases) have been one of the most important issues. On the other hand, limitations related to organ transplantation have made a serious need for advanced therapeutic strategies. In this respect, researchers have recognized the benefits of stem cells as a su...
متن کاملA Two-step Method for the Preparation of Implant Recipient Site in Severe Atrophic Maxilla: A Case Report of the Alveolar Ridge Split Technique Followed by Bone Expansion
Augmentation of deficient and atrophic alveolar ridges is an important aspect of dental implant therapy with the goal of providing a functional restoration in harmony with adjacent natural dentition. Bone splitting technique is considered a distinguished augmentation method for treatment of deficient alveolar ridges. According to this procedure, the compromised alveolar ridge is opened from the...
متن کاملOutcome of short implants as an alternative to sinus lift surgery and standard-length implants for rehabilitation of atrophic posterior maxilla: A clinical trial
Background and Aims: The aim of this study was to assess the outcome of short implants (<8 mm length) as an alternative to sinus lift surgery and placement of standard-length implants (10-12 mm length) for rehabilitation of atrophic posterior maxilla. Materials and Methods: In this prospective clinical trial, 26 patients with 6-7 mm of residual alveolar bone height in their atrophic posterior ...
متن کاملSagittal Distraction of Edentulous Maxillary Skeletal Base Prior to Implant Therapy
Edentulous patients presenting with atrophic ridges and severe prognathic ridge relation are difficult to rehabilitate. Correction of the malrelation of the jaws is an absolute prerequisite for their successful prosthetic rehabilitation. A majority of the established surgical techniques to correct combined sagittal and vertical discrepancies of edentulous jaws are often prolonged and complex wi...
متن کاملAdipose-derived stem and progenitor cells as fillers in plastic and reconstructive surgery.
Plastic surgeons are keenly aware of the principle "replace like with like." This principle underlies much of the rationale behind the clinical use of autologous fat transplantation, despite the procedure's drawbacks. Autologous fat transplantation is frequently used for a variety of cosmetic and reconstructive indications not limited to posttraumatic defects of the face and body, involutional ...
